SCHOOL RESPONSIBILITY – The School agrees to make available to the applicant the use of classroom space, professional diving equipment and supplies (other than personal dive gear required to be furnished by the student as outlined in the School’s catalog), diving station space, diving manual diver’s log, handouts, decompression chambers, and instruction in the course outline in the program designated for the period indicated. The School agrees to award diploma and certificates and to provide assistance in locating employment as outlined in the School’s catalog after the student meets all requirements for the completion. The School reserves the right to schedule subjects within the curriculum in the order deemed necessary and, if necessary, to change or revise class dates. Reasonable and appropriate substitutions in curriculum may be made by the School as deemed necessary or due to equipment failure. The School does not guarantee job placement.

STUDENT PERFORMANCE – The student agrees to attend classes regularly, study diligently, and abide by the rules and regulations of the School. It is understood that failure by the student to attend classes regularly, study diligently, or to abide by the rules and regulations of the School, as stated in the catalog, or as otherwise prescribed by the School, now or in the future, verbally or in written form may result in immediate termination at the option of the School and the School will not be held responsible for the remaining portion of the course of instruction. It is understood that such rules and regulations to be imposed in the future will be reasonable and proper.

TRANSCRIPTS – The School is authorized to request copies of the applicant’s high school, trade school and college transcripts.

WITHDRAWAL – The student may withdraw from the School at any time and the official date of withdrawal (for all purposes, including computing tuition refunds) shall be considered to have occurred on the last day of actual attendance regardless of whether any written notice is received by the School.

REFUNDS – The School’s Refund Policy is contained in its catalog. If eligible for a refund, the student should request such refund either orally or in writing. In the event a request for refund is not received from the student, the School will automatically make a refund to the student according to the terms of the School’s refund policy. All refunds will be made within 30 days after the official date of withdrawal or termination.

FOR THE PROTECTION OF ALL PARTIES, IT IS AGREED THAT NO AGREEMENTS OR PROMISES WERE MADE TO ME OTHER THAN THOSE SET FORTH IN THIS AGREEMENT AND THE SCHOOL CATALOG AND INSERTS, WHICH I ACKNOWLEDGE HAVING RECEIVED AND THAT I HAVE READ AND AGREED TO THE ABOVE APPLICATION AND TERMS AND CONDITIONS STIPULATED. IN CONSIDERATION OF THE STUDENT BEING FROM TIME TO TIME INVOLVED IN PUBLICITY PHOTOGRAPHS AND NAME RELEASES, THE STUDENT CONSENTS AND AGREES THAT THE SCHOOL AND ITS AGENTS AND SUCCESSORS IN INTEREST MAY USE AND REPRODUCE MY NAME AND PHOTOGRAPH FOR ANY AND ALL PURPOSES IN ANY MEDIA. I UNDERSTAND THAT THIS IS NOT A CONTRACT. I FURTHER UNDERSTAND THAT ALL OTHER REGISTRATION FORMS, INCLUDING AN ENROLLMENT AGREEMENT (CONTRACT), WILL BE PROCESSED AFTER THE ACCEPTANCE OF THIS APPLICATION FOR ADMISSION BY THE SCHOOL.
